When picking up your new card, A PARENT/GUARDIAN MUST PROVIDE IDENTIFICATION WITH A CURRENT BROOKFIELD ADDRESS. If you don’t have a driver’s license with your current address, you may bring in mail, a personal check, or a lease/title with the address.

I apply for the right for my child to use The Brookfield Library. I promise to comply with all its rules, to return items borrowed by my child promptly, to promptly pay fines for overdue items borrowed on my child's card, and to reimburse the library for any items which are lost or damaged while in my child's care. I will immediately notify the library of any change in my address, telephone number, and/or email address. I assume full responsibility for materials borrowed by my child.
