Please fill in the following information to apply for an adult library card (ages 14+). You will be notified by email (or phone, if no email is provided) when your card is ready.

When picking up your new card, YOU MUST PROVIDE IDENTIFICATION WITH YOUR CURRENT BROOKFIELD ADDRESS. If you don’t have a driver’s license with your current address, you may bring in mail, a personal check, or a lease/title with the address.
